What: G-W Publisher Local Chapter Advisor Outstanding Service Awards
Who: G-W Publisher will recognize three Local HOSA Chapter Advisors, one from each HOSA region, who have made outstanding contributions of local, regional, state and/or national significance to the promotion, development, and progress of HOSA–Future Health Professionals.
Cash Award: An unrestricted cash award of $2,000 will be awarded the three Local HOSA Chapter Advisors (one from each region).
State Advisor Process, Due by April 15:
- State Advisors nominate one Local HOSA Chapter Advisor from their state for the regional award. State associations will determine their process for selecting one nominee to complete the online application.

The Nomination Process is SIMPLE AND FAST! Information included on the Nomination Form: Nominee’s Name; Nominee’s Email; Nominee’s School; State Advisor’s Name; State Advisor’s Email.

4. HOSA will share the application link with the Local HOSA Chapter Advisor Nominees for completion by the May 1 deadline. A copy of the application for your review only is available here for your review. Click here for the application.
5. Nominees will receive a confirmation email message when the completed application is submitted.
Nominees are current HOSA Local Chapter Advisors and the outstanding Advisors will be selected and awards will be received on the following criteria:
- Exceptional educational talent as evidenced by effective HOSA chapters and student involvement in the classroom and school and/or state and national organizations;
- Exemplary HOSA accomplishments beyond the classroom that provide models of excellence for other HOSA advisors and members;
- Contributions to HOSA are worthy of the spotlight;
- Strong long-range potential for professional leadership; and
- Engage and inspire HOSA presence that motivates and impacts students, colleagues, and the community.

- Description of HOSA accomplishments, achievements, contributions, and involvement (This list should describe HOSA activities at local, regional, state, and/or national levels; chapter and member activities within the school and community, and partnership development.) - What are your greatest assets as an advisor to future health professionals? Share your personal story. Maximum 250 words
